We understand that surgery can feel stressful — for both pets and their people. That’s why we’re here to make the process as smooth and transparent as possible. This consent form confirms your approval for the procedure and outlines important pre- and post-operative care details.

Completing the form in advance helps us stay on schedule and ensures your pet receives prompt, safe care when you arrive.

Dear Client,

Your pet is scheduled for an upcoming surgery. To help make surgery day as easy and stress-free as possible, we’ve put together some simple forms for you to review and sign in advance. Along with this letter, you’ll find the following forms available on our website:

  • Pre-Anesthetic Blood Testing Information Form
  • Surgical Information Sheet

Please review all provided forms carefully. If you have any questions, feel free to call or email the hospital at (305) 386-9909 or stfranciscl@gmail.com. On your pet's surgery day, you’ll need to review and sign an Authorization/Estimate Form. We’ll also confirm a phone number where we can reach you during your pet’s procedure.

The night before your pet’s surgery, please do not provide any food or treats after 10:00 p.m. Water is fine and may be left out. If your pet is taking any medications, vitamins, or injections, withhold the morning doses unless otherwise instructed by the doctor.

At the time of your pet's scheduled surgery, the doctor will answer any remaining questions or concerns and collect the completed Authorization Form.

If you’ve elected to have any recommended blood tests performed, these will be completed prior to the start of surgery (if not already done in advance). Should any questions arise, the doctor may contact you using the phone number listed on your Authorization Form.

After surgery, the doctor will process your payment, review all discharge instructions with you, and provide a written copy. If any instructions are unclear, please don’t hesitate to ask for clarification before or after your departure.
